| An experienced management staff, including an RN Program Director and supervisors are available for referral intake. The program director or supervisor visits and and informs the caregiver and patient ab.out the program, makes a detailed assessment, and develops a care plan with input from the caregiver, taking into consideration the specific needs of the patient. The care plan may include most activities that the caregiver would normally provide. In addition, the Program Director or Supervisor addresses health and safety concerns. For example, the Falls Prevention Program is reviewed along with the home safety checklist. Personalized Homecare employs a staff of more than 80 caregiver nurses aides and state certified home health aides and certified nurses aides to accommodate the case load. These nurses aides are mature, dedicated men and women with diverse ethnic backgrounds, multi-lingual skills, and documented experience in caring for senior citizens. |
